Jonathan Pisani was found 'unresponsive' in his cell
A man who was recalled to prison was found dead in his cell at HMP Forest Bank after just nine days of freedom. Jonathan Pisani, who was 46, was convicted of firearm offences in August, 2021, and sentenced to 15 months in prison.
Mr Pisani was found 'unresponsive in his cell' at 7.05am the following morning and later pronounced dead by a GP. A post-mortem examination gave Mr Pisani's cause of death as drug toxicity, the report said. A coroner gave a narrative conclusion at an inquest into his death held earlier this month. Members of the prison's substance misuse team visited him twice a day to monitor his withdrawal symptoms and to prescribe methadone.
