1/3 Incident Bulletin: HMDC has reported that while completing a pressure test of the high pressure manifold, a near miss occurred on the Hibernia Platform. The manifold valve handle and gear box cap, weighing approximately 27 kg, separated and travelled a distance of 3.9 meters.However, when workers closed the upper diverter valve, the valve handle and gearbox cap – which weigh around 27 kilograms – separated and travelled 3.9 metres.
A barricade was put up and operations were immediately stopped in the area. The C-NLOPB is monitoring an investigation into the incident launched by Hibernia management.
