A New South Wales paramedic stabbed to death outside a McDonald's was a young dad who dreamt of working in the service since he was boy, before his life was cut tragically short while grabbing a bite to eat.

The paramedic, 29, was getting breakfast at Campbelltown McDonald's in the city's south-west with a co-worker about 5:30am on Friday when it is alleged he was repeatedly stabbed in the abdomen by a 21-year-old man.

"I'm told that he was just so pleased that he was going to be able to stay here with his family, bring up his children and look forward to the area he lived in and loved more than anything," he told reporters in a press conference. Police Deputy Commissioner Mal Lanyon said only one person is believed to have been involved in the stabbing who remains in custody and is assisting detectives. The community has been urged to avoid the area surrounding the McDonald's as forensic investigators continue to search for clues and evidence.
