Investigators found the pilot noticed 'engine surging' and 'fuel flow fluctuaions' shortly after taking off from Goote Eylandt Airport.

Shortly after take-off the pilot noticed "fuel flow fluctuations" and "engine surging", before turning around to re-land the plane, according to the Australian Transport Safety Bureau report. "At about 1349:58, the pilot had a brief discussion on the Groote Eylandt Airport CTAF with another pilot that was in the area advising that the aircraft had fuel flow issues," the report reads.

The plane touched down and rolled along the ground before hitting embankments on either side, coming to rest upside down.The pilot noticed "fuel flow issues" shortly after take-off.
