Slain officer Brett Forte’s wife said the Queensland police investigation treated her husband’s death “as if Brett suffered a paper cut to his finger”.

In his final moments, Senior Constable Brett Forte did the only thing he could: reverse his police car to try to save his and his partner’s life.

Handing down his findings following an inquest into Forte’s death, and that of his killer, Ricky Maddison, Terry Ryan told a crowded courtroom there was a “significant failure of leadership” by Queensland police that day.Maddison, wanted over domestic violence offences, had been tracked by police in the months leading up to fatal shooting on May 29, 2017, at Ringwood, near Toowoomba, west of Brisbane.

“No record of the Gatton police investigation ... including the suspected location of the automatic gunfire, was placed on QPRIME,” Ryan said. The officer in the vehicle with Forte, Senior Constable Catherine Nielsen, had said they were “sitting ducks”.In the final seconds of video footage of the incident, released during the inquest into Forte’s death, Nielsen can be heard saying “urgent, urgent” and asking: “can someone help”.As she got her firearm out and started shooting through the windscreen, the car tipped onto Forte’s side. Nielsen realised her partner had lost a lot of blood.
