Todd McKenzie, 40, was in the midst of a psychotic episode when he was shot dead by police at his NSW home in 2019. Deputy Coroner Harriet Grahame has found police failed in their handling of the case in a number of ways.

The findings include recommendations that NSW Police review mental health training protocols within two years. "Once tactical police broke the window and rammed the door of Todd's home, disaster of some kind, in my view, was almost assured." She also suggested the police commissioner update operating procedures to ensure team leaders kept records regarding their decisions about the use of consultant psychiatrists in operations."Todd was a beloved son, he was a young man who wouldn't hurt anyone," he said.Mr Wilkins called on NSW Police Commissioner Karen Webb to meet with the family and apologise for the incident.
